About this website

This website provides information about Priyal Physiotherapy and lets you send an appointment contact request. This page describes information handled by the website; it is not the clinic’s clinical-record privacy policy.

For a question about your website request or the clinic’s information practices, call 289-633-5454 or email info@priyalphysiotherapy.ca. Please keep medical information and identity documents out of an initial email.

Browsing and technical requests

Pages are delivered through Sites hosting. The hosting service may process technical information needed to deliver and protect the site, such as IP addresses, request details and security logs. Its account services provide sign-in for the private clinic inbox.

The website application does not add analytics, advertising pixels, session recording, newsletter signup or marketing cookies. No optional tracking is enabled.

Appointment contact requests

The form asks for your first name, last name, phone number, an optional email address, your preferred contact method and an optional short reason for the visit. An email address is needed when email is your preferred contact method. Please avoid including detailed or sensitive medical information.

Submitted details are saved in the website’s Cloudflare D1 database so the clinic can review and follow up. The record also contains a technical request identifier, receipt time and follow-up status. Access to the inbox is restricted to the configured clinic administrator account through the hosting service’s sign-in.

Submitting the form does not confirm an appointment. The clinic reviews requests manually; this form does not send automatic email or text notifications. It does not store your submitted contact details in browser storage or page URLs.

To ask about access to, correction of, retention of or deletion of a website request, contact the clinic using the details above. No automatic deletion schedule is implemented on this website.

Movement Guide and visit checklist

Movement Guide selections and the Movement Snapshot are handled in your browser. They are not sent to the clinic as an assessment or an appointment request. You choose whether to print or download a snapshot.

The first-visit checklist can be used in your browser and printed. Keep any personal notes you add to a printed or downloaded document private.

General email enquiries

Selecting an email link opens your own email application. A message is sent only when you choose to send it. Ordinary email is not a clinical-record service; please do not include medical history, clinical documents or sensitive identifiers.

Booking, referrals and outside links

Instant online booking is not active. An appointment is confirmed only when the clinic agrees a date and time with you. This website does not collect payments or receive clinical referral documents.

Directions links open Google Maps only when selected. College registration and educational source links open their respective providers. Those services handle your interaction under their own policies. No embedded map or video loads automatically.
